Fundraising Agreement

In consideration of the acceptance of my registration in the 2004 Choo Choo Challenge (“CCC”), I agree to raise a minimum of $250 for the benefit of Memorial Hospital’s Cancer Resource Center. I understand that the funds must be submitted to the Choo Choo Challenge by July 30, 2004.

Cancellation Policy

I understand that my registration fee of $100 to participate in the Choo Choo Challenge is non-refundable and if I cancel at any time I will forfeit the $100 registration fee.

I have read this fundraising agreement and cancellation policy, fully understand its terms, understand that I have given up substantial rights by agreeing to it and agree to it freely and voluntarily without any inducement.

The registration fee of $100 must be paid before your registration is accepted. The registration fee is not refundable and not transferable. The fee is not part of the fundraising and therefore will not apply towards your fundraising minimum.
